Approval Requirement Not Working

In the Group settings, I have set up my gallery so that every group, except Administrators, need approval before their uploaded image can be viewed. I have set "yes" for both public and private upload approval. Yet, all registered users who come to my site are able to upload without approval. (I haven't checked what happens if a non-registered or anonymous user uploads a file.)

Is there some other setting that is required in order for the approval process to work?

What version are you running?  Have you done any modifications?  Do you have your gallery bridged to a bbs?

Please post a link to your gallery along with a test user/password.
