[SOLVED] major errors on install

I've installed 1.1d before so I'm really lost.  

When I First run install I get the screen where it asks for the table prefix.

After I press the install button I get the errors below.  I confifmed that I unzipped cpgnuke1.20rc2.zip.

I guess changing it to 777 got rid of the error.  Why does the instruction say 766?

Does the sql dir go in the nuke root dir? or in the modules/coppermine dir?
