Registered users unable to upload files

I am running Coppermine 1.4.3. I have gone through most of the available info here and find that this problem is still present.
Debug mode is ON, single file upload only (as per instructions on this board).

The URL is: http://www.dutts.selfip.com/index.php user:test2 password:test2

OS WinXPSP2 Pro, Apache 2.0.54 php 5, mysql 4.1

Any pointers where I am going wrong?

It might be that the specific user was not doing the upload process properly.
