THe previous file could not be placed

i'm running the latest version.  the only way i can get it to work is by setting "file upload boxes" to 1 and "URL upload boxes" to 0.  if i try any other combo of settings under "groups" it will not upload.  Any ideas?  i have been through the forums on this plus i did do the debug thing as instructions says to do.

Re-read the instructions in the upload troubleshooting section of the docs extra-carefully and do exactly as suggested there.
