upload issues

Hi, I am trying to upload pics as administrator on the following gallery url.
http://www.evereadygroup.com/gallery

When I enter filename or URL, and click upload, it shows File upload was successful, then after pressing continue, it asks for further information about the images with its thumbnail. after filling the form, on next page it says. The previous files could not be placed.

FYI, the hosting server is windows 2003.

Please advise.
