Hide categories (smart_catlist) Plugin not working

Hello All,

I am running Coppermine v1.4.25.  I uploaded the smart_catlist plugin via FTP and then installed it via the config page, but it does not appear to be working.  When I go to the categories page, nothing is different.  I also tried to install it by uploading from the plugin manager page but it never uploads.  The page just refreshes.

I verified that the config has the option enabled to allow plugins.  I also read the other posts but nothing seems to really match my problem.  I turned on Debug and the following was displayed:

/catmgr.php
    * Notice line 433: Undefined variable: description_help

Any help would be appreciated!
