Fix media player HTML plugin

This plugin modifies the HTML generated for the HTMLV and QT media players. For QT, it adds the important <embed> specification to allow playback in more browsers. For HTMLV, it just removes the "width" parameter, allowing the player to size the display more naturally.

These are things that should probably be taken care of in the Coppermine core ... but, for now, here is this plugin.

Off-topic: I thought playback of QT files has been fixed since some Coppermine versions?

Thank you for this. I downloaded the file but there is no readme or instructions with it. I see two php files but where am I supposed to upload them to? Thanks.
