Problem with MP4 and plugins?

I have been trying to upload an mp4 file to cpg1.6 and have installed several plugins including file_types to which I added an mp4 type. Suddenly my Coppermine display reverted to some older version as shown in the attachment. Please help. Thank you

ron4mac, thanks again. What did I do to mess things up? I want to avoid making the same mistake again. Was it one of the plugins I tried?
