Okay, I get that the organization starts with categories, then inside the category are albums. Is there a way to create a sub album within an album?
Also, I would like for the index page to be layed out like this: http://www.clooneynetwork.com/photogallery/
Is that possible?

You can't have sub album but you can have Sub category

Yes, you just need to create your custom theme and set index layout under config
