By hosting provider made some "changes" to the server and knocked my Coppermine photo gallery offline for a few days; now that it's back up it doesn't really seem to have any formatting or background (theme?) ... could someone take a look and suggest a way to fix it?

Check if they moved your site to a different location on their server. The problem originated with them working on their server, they should at least be able to solve your problem if they are a good hoster.

And you might want to upgrade to cpg 1.4.16

Yes you can but it is always wise to backup before performing any upgrade.

And tell your hoster to wake up, the problem apparently started when they were fiddling with the server.

Delete the template.html from your gallery root.

Upon deletion of the specified file... IT WORKS!!!!
