I turned intermediate photos off, and then uploaded pictures, and turned it back on... now the pictures do not have intermediates and show up broken. is there a button to hit to recreate intermediates for photos that do not have them, or recreate all intermediates?

Yes. In the admin menu you will find a link called "Resize pictures". If you open this there is an option to rebuild sized (intermediate) photos. You have to do it one album at a time.
