Anyone have any thoughts?

pn v7.6.4

Which version of pnCPG are you using ?
Please use latest version (3.5)

Where did you copy index_pn.php ? It should be in the root of your coppermine installation. looking at your messages you have pointed the location of Coppermine to be the location of pnCPG. This is wrong, see the docs.
