Unable to Upload Problem

I have tried to upload pictures to no avail.

I have debug mode on and when I click on upload it displays

Notice: Undefined offset: 4 in /home/gkochane/public_html/coppermine/upload.php on line 256

which the code for these lines are respectively

If I choose a picture anyway and try and upload I get this

I have no idea what any of this means??
