Report to Admin - two different bugs

1. In mails recieved from CPG (created in in report_file.php form) there are incorect values in positions: FROM and TITLE and wrong formatiing message area (in v.1.4.1 beta it was correct).

Text in FROM position:

2. In report_file.php form I've got an error message each time, I try to write anything in MESSAGE textarea. In moment, I place cursor inside textarea and click, then I have the message (free translation from polish):
"There is an error on this side; line: 282; sign: 1; error: it should be an object; Code: 1"

I could not replicate either issues using 1.4.3, even when using polish. Please create separate threads for different issues to make it simpler to track them. Please post more details, like links and message headers. I'm locking this thread.
