Upload - Title and Description do not get inserted in the DB?

When I try to upload a file the Title and Description fields only show up on the upload form if I'm a Guest and when I fill them out they do not get inserted in the DB? When the approval sceen comes up they are blank. Anyone help?

Checked and re-applied the permissions on all folders in "Album". User "tester" Pw "tester" at www.mimawscloset.com/gallery [NSFW]. Also the "Title" and "Description" fields only show on the Upload form for guests. Thanks

Still shows 1.5.18

If you have any plugins enabled please disable them one by one and see if that changes the form.

No plugins installed. Something go wrong with the update maybe?

Maybe you downloaded cpg1.5.18 instead of cpg1.5.20:

I chose re-install 1.5.20 instead of updating. I still have a similar problem only  now "Title" and "Description" fields don't show on the "Upload" form for guests but do for administrators and registered users. When I first tested this guests and registered users worked fine but administrators did not. I'm so confused!

Those fields arn't suppose to show for guests, it's expected behavior.
