Registered User Uploads Error

Can someone please tell me what is going on ?

I have two users (this is a brand-new gallery) that are trying to upload their photos. They can create an album but it's when they try to upload a file that the error occurs.

Once the file is chosen and uploads, it shows # of uploads successful. Click 'Continue' to add files to albums ....

When the Upload File screen comes up with the uploaded image and you select the album to place the file(s) in, this message comes up: The previous file could not be placed.

What is going on 

I've checked all permissions and everything seems set, but they can not upload ......
