Need and 'Opt in' Checkbox on profile page

I am needing a checkbox located on the profile page. I have a newsletter I send out and I want users, upon registering, to have a choice to opt-out via a checkbox at the bottom of the profile window.

I also need the true/false results sent in the admin email.

such a checkbox is integrated into the modpack

there's a newsletter plugin that makes use of it

but there's no checkbox at the register page. You'll have to write that into your disclaimer
