Installation CF-error and sample images not displayed

Hello,

I am trying to install release version 2 of cpmfetch.  The cpmfetch.php file says that it is Revision: 1.12 if that is useful.  I also have Coppermine 1.4.11 running.  My image gallery seems to work fine. 

When I attempt to run the installation of cpmfetch I see the following:

The 'images' are displayed as just the names with blue boxes around them, as noted in some other posts experiencing the same problem.  The number of images that it found and the four file names are definitely correct. 

I placed the cpmfetch folder in my coppermine directory and changed the file permissions on the files indicated in the documentation.  I don't believe I have a path problem but, honestly, I have spent so much time on this that I am starting to second guess myself. 

I am sorry to be posting the same issue that others are seeing, but those posts don't really seem to be going anywhere.

Here is a dump from the installation (I have tried this several times so it shows a forced installation).  Hopefully this is useful and not just an annoying waste of forum space...
