When I login as administrator I can ban users.
But how to do that. I got three text boxes. Can I ban users when I have only their IP address?

But the user had to be a registered user?
The user I like to ban isn't a registered user. I've only his IP address. Filling in this Ip address I got the message: Cannot find user <IP address>
So I think the user had to be registered. Is this correct.

You need to put the IP address in the box that says 'IP Address'.

There's a field for the username, and a field for the IP address - fill in the correct one.

Sorry, but all what I see is three empty boxes and a button with the text "Add".
Perhaps this is the problem. What do I wrong.

It is the middle box.  The reason you cannot see it is because of your theme, but if you look hard, it is there, in the dark portion above the boxes.  I can just see it on your screenshot.
It has been a long time now since I did my little bit here, and have done no coding or any other such stuff since. I'm back to being a noob here

and btw: since nearly all users have dynamic IP addresses, banning a user by IP does make very, very little sense. Next time he connects to the internet, he'll probably have another IP address assigned from his ISP, so your banning actually won't do you any good.
IP banning only makes sense if people use static IP adresses to access the internet (this is most often the case for people surfing from companies or universities), but then you'll most likely not only ban the particular user, but the whole company or organisation he's in.
Also make sure not to ban yourself accidentally!

In other words: don't use banning if you're not sure how this works...
