Yes, I read the docs.  I also searched the forum postings, and saw other postings by people who could not get custom thumbnails to display for their videos.  I also cleared my browser cache several times, and even accessed the album page from a different computer.  Still, I cannot get a custom thumbnail to show for my video.

reupload the movie with the extension all lowercase
